Physician Assistant | Family Practice Location: New York Employer: Lancesoft Inc Pay: Competitive weekly pay (inquire for details) Start Date: ASAP About the Position We are seeking a Primary Care Nurse Practitioner or Physician Assistant to join a collaborative, patient-centered primary care practice in New Windsor, New York . This role is ideal for an APP who is passionate about preventive care, chronic disease management, and long-term patient relationships —all within a supportive and well-structured care model. Role Overview As a Primary Care APP, you will function as a licensed, independent provider within a team-based primary care model , delivering comprehensive care across the lifespan. This position is 100% primary care —no specialty or surgical responsibilities. Clinical Responsibilities Comprehensive Primary Care Preventive care, wellness visits, acute illness evaluation, and chronic disease management Perform complete and focused histories and physical exams using evidence-based guidelines Diagnosis & Treatment Diagnose and manage common primary care conditions Prescribe medications and therapies within scope of practice Order, interpret, and follow up on labs, imaging, and diagnostics Patient Education & Care Planning Develop and monitor individualized care plans Emphasize prevention, lifestyle modification, and patient engagement Collaboration & Care Coordination Work closely with physicians, nurses, MAs, and care coordinators Participate in daily huddles and interdisciplinary care planning Coordinate referrals and follow-up care ️ Documentation & Quality Document patient encounters accurately in the EMR Utilize population health tools to support quality metrics Adhere to clinical guidelines and billing standards Position Highlights ️ Choose a 4- or 5-day workweek ⚖️ Strong focus on work-life balance Optional AI scribe support to reduce documentation burden Collegial and supportive environment across 35 locations Compensation Base Salary: $120,000 – $130,000 Productivity & Quality Bonus: Up to $5,000 Benefits Package CME Allowance: $1,500 3 paid CME days ️ PTO: 2 weeks 6 paid federal holidays 401(k) with 6% employer match ️ Malpractice insurance coverage ❤️ Health, Dental & Vision Insurance Life Insurance ✅ Candidate Requirements ✔ Nurse Practitioner or Physician Assistant degree from an accredited program ✔ Board Certified in Family Medicine or Internal Medicine ✔ Active, unrestricted New York State license ✔ DEA license (or ability to obtain) ✔ Current BLS/ACLS certification ✔ Clean malpractice history Why New Windsor, New York?
